Introduction: The insertion of peritoneal dialysis catheters (PDC) is prone to complications such as displacement, blockage, and leakage. Laparoscopic insertion was reported by the senior author in 2016 and adopted in two linked tertiary centers. It offers the advantage of subfascial tunneling and controlled positioning of the tip, making it is less liable to migration and leakage. This is especially important following previous laparotomies. This work evaluates the outcomes of laparoscopically inserted PDCs in two tertiary centers over the last 2 years, combined with a video presentation of the technique. Methods: Thirty peritoneal dialysis catheters were inserted from August 2016 to August 2018. The procedure included an omentectomy either performed laparoscopically or by delivering the omentum to the umbilical port site (15 cases each), followed by a laparoscopy-guided properitoneal PDC insertion using the Seldinger technique.

However, because of a high risk of complications with prolonged use, these catheters are not commonly used. A peritoneal dialysis catheter typically lasts about 3 years before needing replaced. We used a laparoscopic-assisted method for placement of the PD catheter to ensure proper placement and function. The procedure includes an omentopexy, which means that the omentum, a fatty apron-like fold of visceral peritoneum that hangs down from the stomach, is put away from the catheter, this prevents the omentum from wrapping around the catheter, thus, obstructing it.
