Geneva, World Health Organization, 2019. The maternal death rate varies greatly by race and ethnicity worldwide. In the United States, the maternal death rate is over 3 times higher in black women and 2. 5 times higher in American Indian and Alaska native women than in white women. In Brazil, the maternal mortality is about 5 times higher in women of African descent than in white women. In the United Kingdom, it is many times higher in black women than in white women.
The team may include an obstetrician (who may also be a specialist in the disorder), a specialist in the disorder, and other health care practitioners (such as nutritionists). During pregnancy, a problem may occur or a disorder may develop to make the pregnancy high risk. Some disorders that occur during pregnancy are related to (are complications of) pregnancy. Other disorders are not directly related to pregnancy (see Pregnancy Complicated by Disease). Certain disorders are more likely to occur during pregnancy because of the many changes pregnancy causes in a woman's body. Pregnancy complications are problems that occur during pregnancy. They may affect the woman, the fetus, or both and may occur at different times during the pregnancy. For example, complications such as a mislocated placenta ( placenta previa) or premature detachment of the placenta from the uterus ( placental abruption) can cause bleeding from the vagina during pregnancy. Women who have heavy bleeding are at risk of losing the baby or of going into shock and, if not promptly treated, of dying during labor and delivery.
Many of the more complex organs such as the brain and heart cannot be fully evaluated on ultrasound until later in the second trimester, around 20 weeks. It is around this time that a detailed ultrasound is performed to evaluate for birth defects. If a major anomaly is encountered, and an abortion is performed/requested, it's not a pleasant process. These situations can be more challenging on a personal level. However, I feel a personal responsibility toward my patients, which is why I made a personal decision that if I was going to be performing abortions, I would let patients make the decision for themselves and my role would be to support them. I knew that if the majority were having very desired pregnancies (which they are), but for whatever reason could not move forward with the pregnancy, I wanted to be able to take care of them rather than have to refer them out to other clinics. What Has Surprised Me One of the things that strikes me most about my mentor, who trained me to perform D&Es, is that he's saved more pregnancies than he's terminated.

A high-risk pregnancy is one in which a woman and her fetus face a higher-than-normal chance of experiencing problems. These risks may be due to factors in the pregnancy itself—such as an issue with the fetus or the placenta—or they may stem from preexisting maternal medical conditions, such as cancer, diabetes, or lupus.
If you smoke, quit. Alcohol and illegal drugs are off-limits, too. Talk to your health care provider about any over-the-counter and prescription medications or supplements you're taking. Do I need special tests? Depending on the circumstances, your health care provider might recommend: Specialized or targeted ultrasound. This type of fetal ultrasound — an imaging technique that uses high-frequency sound waves to produce images of a baby in the uterus — targets a suspected problem, such as abnormal development. Prenatal cell-free DNA (cfDNA) screening. During this procedure, DNA from the mother and fetus is extracted from a maternal blood sample and the fetal DNA is screened for the increased chance of specific chromosome problems. Invasive genetic screening. Your health care provider might recommend amniocentesis or chorionic villus sampling (CVS). During amniocentesis, a sample of the fluid that surrounds and protects a baby during pregnancy (amniotic fluid) is withdrawn from the uterus.
"I always tell my patients a healthy mom is so beneficial for the baby, " says Shannon Clark, MD, Ob/Gyn MFM at the University of Texas Medical Branch in Galveston, Texas. "If you came into the pregnancy with poorly controlled diabetes, it's going to be a complicated pregnancy. If you come into it with well-controlled diabetes and you maintain that, the chances are your pregnancy is going to be just fine, " she says. "All patients should be in optimal health before they get pregnant and that's particularly true for women with underlying medical problems, " adds Dr. Pressman. 3. "Age is one common high-risk factor we can't control. " "Anybody over 35 has a high-risk pregnancy. Especially if you are over 40, there's a higher chance of having miscarriages or trisomy–three instead of two of certain chromosomes–which results in Down syndrome, " says Jan Rydfors, MD, an Ob/Gyn at the Freyja Clinic in Redwood City, Calif., with extensive experience in high-risk obstetrics. "Age increases your risk of having gestational diabetes and preeclampsia, and there's an increased risk of stillbirth as you get close to the due date, " he says.
Preconception genetic screening has become more common in recent years as technology has advanced and testing has become more accessible. If you have family members with a certain disease or if you belong to an ethnicity that has a greater risk of developing specific conditions (such as sickle cell disease or Tay-Sachs disease), genetic screening can be used to assess your and your partner's risk of being a carrier. Also, common genetic conditions, such as cystic fibrosis or spinal muscular atrophy, can be screened for with a blood test. Additionally, if you have a child affected by a genetic disorder or syndrome, a maternal-fetal medicine specialist can provide counseling and management to consider how the condition may impact your future pregnancies. Always speak with your health care provider to determine what's best for you and your pregnancy. Q: What conditions may lead to a high-risk pregnancy?
